Note from the Provider: If it is determined that you have ADHD, I will not prescribe any stimulant medications (Adderall, Vyvanse, Concerta, etc.) under the following conditions:

If you are using any illicit substances including any form of cannabis even if it is prescribed (medical marijuana) or abusing alcohol.
If you are pregnant (or intend to become pregnant) or breastfeeding.
If you have been abusing stimulant medications including having a history of asking for early refills, lost/stolen prescriptions, etc.
If you have (uncontrolled) high blood pressure, tachycardia, other cardiac (heart) problems, or any other medical condition that is contraindicated for stimulant medications.
